Below are the Candidate and Sponsor application forms. Both forms will need to be filled out for each candidate. There are detailed instructions regarding sponsorship included with the Sponsorship form. Each Sponsor should carefully read these instructions before you approach your potential candidates about attending a Cursillo weekend.
